Advance Directive Members $25.00
Our Advance Directive Management Service will store your notarized documents and make them available to health professionals or a designated family member/ healthcare proxy in the event of an emergency. We also provide the official advance directive forms recognized by each state.

Medical ID bracelet for children. General Warranty

MedicAlert Foundation® offers a one-year warranty from the date of shipment against manufacturing defects in materials and workmanship for all 14K Gold, 10K Gold Filled, Sterling Silver and Stainless Steel emblems, chains and findings (excluding beaded or strung products). Defective products may be returned within the warranty period for replacement or repair. Warranty claims will be reviewed individually and given a prompt response.

Warranty Exclusions

General warranty does not cover: normal wear; tarnishing or scratching of the emblem, chain and attachments; damage resulting from misuse or accidental handling; or loss of any kind.

Warranty Service

Should your MedicAlert medical ID have a manufacturing defect or faded engraving within the warranty period, it will be repaired or replaced and returned to you free of charge. For warranty service, securely package and mail your ID via insured freight to: MedicAlert Customer Satisfaction Department, 2323 Colorado Avenue, Turlock, CA 95382. Please include your name, ID number, mailing address, phone number and service you wish to receive.

Non-Warranty Service

Should your MedicAlert medical ID require repair, replacement or resizing and is not within the product warranty period, MedicAlert offers repair services for nominal fees. Shipping fees will apply. Please call Member Services at 1-800-432-5378 to request a quote.

Allergic Skin Reaction Warranty

Members reporting allergic skin reactions may exchange their MedicAlert product for a more suitable replacement within 30 days of shipment.

Engraving Warranty

Personalized engraving is warranted against fading for one year from date of shipment.

Resizing Warranty

We offer free resizing of all chain link bracelets within 30 days from the shipping date. After 30 days a resizing fee and return shipping fee will apply. Resizing is not available for the following products: Stretch Band Bracelets, Glass Beaded Bracelets, Swarovski Pearl/Crystal Bracelets, Sports Bands, Leather Bracelets, Watches and Necklace Chains. Members are responsible for mailing their product to MedicAlert Foundation for resizing. For resizing service, securely package and mail your ID via insured freight to: MedicAlert Customer Satisfaction Department, 2323 Colorado Avenue, Turlock, CA 95382. Please include your name, ID number, mailing address, phone number and service you wish to receive.

Citizen Watch Warranty

We offer a one-year engraving warranty for the Citizen watch. Additionally, a limited five-year warranty for the watch, band, and components is provided by Citizen Watch Company.

Roots Watch Warranty

We provide a 1 year warranty against manufacturer's defects on the Roots watch nylon wristband. Additionally, a limited two-year warranty is provided by Roots Company, limited to the watch movement, dial and hands. Regular wear-and-tear and battery life are not covered under the warranty.

Silver Tarnishing Warning

We have observed that a high percentage of diabetics and individuals taking an anticoagulant have experienced tarnishing of the non-coated silver products. For these members, we recommend selecting rhodium-coated silver product or another metal type. MedicAlert Foundation will not warranty against tarnishing of non-coated silver MedicAlert IDs for these particular members.

Whether you're ordering a bracelet, watch, or sports band, it's important your MedicAlert® emblem fits comfortably around your wrist. Please use this information to select your correct bracelet size. If you're ordering a necklace, dog tag, or other MedicAlert product, look at the item's "product specifications" for sizing information.

To determine your size, snugly wrap a tape measure around your wrist. Note the measurement, then add half an inch. This is the size MedicAlert bracelet you'll need.

Most of our metal chains are customizable in half inch increments. Sizes range from 5 to 10 inches. Stretch bands, sports bands, watches, and beaded bracelets come in set sizes. You may view their available sizes under the item's "product specifications".
