FAI Partners with MedicAlert

The Food Allergy Initiative (FAI) and MedicAlert Foundation are proud of their ongoing partnership to provide free enrollment for low-income children and adults who suffer from food allergies. Members select a MedicAlert medical ID, which is engraved with their member number and our 24-hour emergency response number. This information saves lives by giving emergency medical personnel immediate access to the member’s medical records. This offer of free MedicAlert membership to needy individuals is made possible by a generous grant provided by the Food Allergy Initiative on behalf of an anonymous donor.

To be eligible for sponsorship through the FAI program, individuals must have a food allergy and qualify through the following means:

1. A completed MedicAlert enrollment form
2. A brief statement from member or person requesting sponsorship due to financial circumstance
3. Proof of Hardship (one of the following):

• Government, state or federal, assistance program such as public assistance card, Medicaid, Medi-cal in California, food stamp program
• Income Tax Return
• A letter or brief statement from a doctor, nurse, school nurse, social/case worker or homeless shelter on letterhead (physician’s prescription pad is acceptable), attesting to the person’s inability to afford the membership fee, renewal or reorder
• Social Security Disability Insurance

Program Limitations

As a nonprofit organization, MedicAlert Foundation reserves the right to modify this program to assure fairness in its implementation as well as adequate annual funding. Although MedicAlert Foundation’s intent is to extend sponsored memberships to anyone who needs our emergency medical information service but cannot afford it, the number of memberships sponsored annually must be limited to the resources available from donations received on behalf of this program.
